樱花视频 Department of Health Awarded $8.5 Million CDC Grant For Prevention and Management of Diabetes, Heart Disease And Other Chronic Diseases

The Department of Health (DOH) today announced that it will receive $8.5 million in grant funding from the federal Centers for Disease Control and Prevention (CDC) to support state investments in implementing and evaluating evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in disproportionately affected populations.

“Heart disease, stroke and diabetes are among the leading causes of death for both the nation and 樱花视频,” said 樱花视频 Health Commissioner Dr. Shereef Elnahal. “This funding will be used to build upon ongoing efforts to lower the burden of disease among 樱花视频 residents.”

In 2016, heart disease, stroke, and diabetes accounted for more than 20,000 deaths in the state.

Since 2013, the Department’s chronic disease programs have provided more than $10 million in funding to community-based and healthcare organizations and public and private agencies to increase access to diabetes prevention and self-management education programs and increase reporting, monitoring and tracking of clinical data for improved identification, management and treatment of at-risk patients.

The new funding will be awarded to successful applicants following a Request for Applications (RFA) process later this year.
